The more readily available your documentation is, the faster you’ll move through the process.
In order to apply, you will need to submit:
- Last 3 months of Bank Statements
- Last year’s filed tax returns
- Award letters showing disability, social security, or other fixed income.
- Other documentation supporting hardships

Last 3 months bank statements
If you are on social security, retirement, disability etc. We need proof of this.
LAST YEAR'S TAX RETURNS OR 4506-T
We require the last year’s filed tax returns to make a determination of what was earned by you.
Any other documentation that supports your claim of a hardship.
